Abstract. Prostate cancer is among the most biologically heterogeneous malignancies affecting men worldwide and presents substantial challenges in prevention, diagnosis, risk stratification, and treatment. While many tumors exhibit indolent behavior, others progress aggressively and contribute significantly to cancer-related mortality [1]. This variability complicates clinical decision-making and highlights the importance of personalized approaches to disease management. The complexity of prostate cancer results from multifactorial interactions involving genetic alterations, hormonal regulation, tumor microenvironment dynamics, and evolving molecular pathways [2]. Advances in imaging technologies, genomic profiling, and biomarker discovery have improved diagnostic accuracy; however, controversies related to screening, overdiagnosis, and overtreatment persist [3]. This article examines the biological, diagnostic, and therapeutic complexities of prostate cancer and discusses emerging strategies designed to enhance precision medicine and optimize patient outcomes.
